Shipping Insurance Claims Form

 

  • SHIPPING PROTECTION
    • Coverage in case your package is damaged in transit, stolen, or doesn’t arrive.
  • INSTANT ISSUE RESOLUTION
    • Get a refund or replacement by emailing Beautifully Unique Leggings with your order #, along with a description and photos of items pertaining to the claim and reason for the claim (what happened to your package).
  • CLAIM SUPPORT
    • Beautifully Unique Leggings (BUL) Shipping Insurance extends coverage for online purchases that are lost or damaged in transit, or stolen immediately after the carrier’s proof of delivery where BUL traces the transportation. BUL is the named insured on the shipping insurance policy; BUL Shipping Insurance is neither returnable nor refundable.
    • All claims must be filed 30 days from the last tracking update for lost orders and 30 days from delivery date for damaged or stolen orders. If a claim is filed outside these time frames, BUL reserves the right to deny the claim. 

 CLAIM PROCESS:

  1.  Fill out the Beautifully Unique Leggings Insurance Claim Form. Located here.
  2. Email Beautifully Unique Leggings . Include: Your Insurance Claim Form. For orders containing multiple items, you may file a single claim if the entire order was affected by the same issue, or multiple individual claims if only certain items were affected.
    1. Your order #.
    2. A description and photos of items pertaining to the claim.
    3. Specify your resolution preference.
